Relevant records
Bring prior neurological notes, reports, and test results.
Medication information
Prepare a list of medications, doses, allergies, and recent changes for the clinical team.
Your observations
Note seizures, passing out, inattention, staring events, clumsiness, unexplained memory decline, nighttime (sleeping) events, or other concerns. Include timing and what a witness noticed.
Changing an appointment
If you cannot keep an appointment, please call the practice with at least 24 hours’ notice so we can update the schedule.
